Lincolnshire Co-op pharmacies offer a prescription collection service, so that our customers can collect their prescriptions directly from a pharmacy of their choice, without the need to visit a GP practice first.

Customers can now sign up for this service simply by filling out a quick form online. This makes it easier, quicker and safer for you to request collection from your nominated pharmacy.

Once you have completed the form below, you don’t need to do anything else. You can order your medication in the same way and your GP will send your prescription electronically to your nominated pharmacy, where it will be prepared ready for collection. Once it is ready, we will send you a text message informing you that you can pick up your prescriptions.

Some pharmacies may provide delivery services. You are advised to check with your local branch and discuss your delivery needs with them.
